The area around the river is very flat and ideal for farming. The river can be broken into three different stages, the upper reaches, middl e reaches and lower reaches. This has become a reality for many parents as they find out that their child has a form of muscular dystrophy. This particular type of muscular dystrophy is referred to as Duchenne Muscular Dystrophy, which is a genetically inherited disorder that is the most common and severe of all muscular dystrophies. It is found in 1 of every 3,500 males and is characterized by progressive muscle wasting. It is caused by the lack of dystrophin, which is a protein found in the cell membrane of muscles. One early clinical sign of the disease is the child showing a late start in walking and sometimes they are referred to as a late bloomer. Usually when the child walks they have a waddling motion and sometimes they walk up on their toes. Usually, hypertrophy, or enlarging of the muscles, especially in the calves is noticed. However, the hypertrophy is actually caused by adipose (fat) tissue, which replaces the muscle tissue. Another clinical sign of DMD is small amounts of mental retardation or learning disabilities. Although this is not apparent until the child is older, it is a common pattern that is found in children with DMD. There have been many studies done to determine what exactly causes the mental retardation. Duchenne’s Muscular Dystrophy is a recessive genetic disorder caused by a deficiency of the protein dystrophin, which is found in muscle cells. This disorder is usually only found in males. It has very rarely been found in females because they have two X chromosomes and the disease is recessive. They pass it on to one half of their sons and to one half of their daughters who become carriers. In the possibility that the male with DMD reproduces, his sons would not be infected however is daughters would be 100% carriers of the disease. Even though DMD is present from the initial stages of fetal development, physical signs are not present until 18 months to 4 years of age. In addition to the clinical signs mentioned above, a child with DMD is unable to climb or pull itself up from the floor. Between the ages 3 and 7, a child with DMD may show signs of improvement, but that is due to natural growth and development. In school they start to fall behind in being able to keep up with their peers in physical activity. This sometimes leads to them being called clumsy or lazy. When the child is around 9 years, the disease starts to progress rapidly and by age 12, they can no longer walk on their own. In the late stages, a significant loss of muscle tissue is noticeable. There may also be an increase of the curvature of the spine. When they are brought to the doctor, a series of tests are done to determine the problem. When a biopsy is done, the doctors find that the muscle tissue has hypertrophied with adipose tissue. Short ly after the cells have become full of adipose tissue they die. Unlike other tissues of the body, muscle and nerve tissues cannot reproduce. An individual is born with a specific amount of muscle and nerve cells. Once they die, they cannot be regenerated. This is bad news for people with DMD and they usually die around the age of 20 due to the diaphragm or heart muscle degenerating. There are several ways to test for Duchenne Muscular Dystrophy. The first is a blood test, called the serum creatine kinase test, looking for an increased amount of creatine kinase. Normally there is only a small amount of creatine kinase in the bloodstream. All muscles produce creatine kinase and release small amounts, however when the muscle cells tear from contracting, the creatine kinase leaks into the bloodstream. The second test that could be done is an electromyopathy test. This involves putting a small needle in the skin and recording the pattern of electricity. When muscles contract, there is an electric current flowing through the tissue. The third way to test for the disease is a muscle biopsy. In this process a piece of muscle is removed and examined under the microscope. When the cells are seen as enlarged and filled with adipose tissue then the disease is prevalent. Or another sign is if the muscle cells are dead. The last way to test is direct DNA testing. While the c hild is still in the womb it can be tested for the disease by doing an amniocentesis. They called America and USAS clash the Cold War which began mainly due to America and the USSRS political differences. The USSR was a communist state and the USA and the other countries who were their partners were capalist states or countries. Many people believed that capalists and communists could not live alongside each other for long and that one system would take over another, however both sides were determined not to be taken over. This is how it all started as the governments were trying to take over large parts of East Europe and Asia. When the buffer zones were added there was a greater risk of war as the zones were dominated by the USSR who were causing a government domino effect through to the west so the USA and their partners were determined to stop it which caused the Truman Doctrine. The main reasons for the USA to get involved with Vietnam were because of their fear of communism and that communism could take over the Western world and Asia. Another reason why the USA didnt like communism was because in 1945 the USA built and tested the first nuclear bomb on Japan but kept it secret from the Russians even though they were supposed to be allies. This made the Russians very suspicious of America. America also didnt like communism because of their very different political beliefs as the communists were a totalitarian state whilst the USA believed in democracy and were afraid the Russians would try to spread their beliefs around the world. The first way of the USA to get involved was to supply France with money as during World War 2 they had lost control of North Vietnam however managed to keep control of South Vietnam. The USA paid the French armies to regain control of North Vietnam so to keep it capitalist and was thought to be a stand against world wide spread of communism. However in 1949 Americans were worried because China had turned communist and supplied money and weapons to North Vietnam making the war harder to win. The French assumed that they were invincible as they were surrounded by mountains and an airbase but the French miscalculation led to a humiliating defeat and ended French plans to regain control of Vietnam. This meant the USA had to get more involved as  well as take more action as communism continued to get stronger. The USAs next involvement plan was to send military advisors to South Vietnam. In 1954 Eisenhower gave 17 officers sealed orders sending them to Saigon and by January 1961 the number of advisors had grown to 685. However the advisors were not there to fight but to train the Army of the Republic of Vietnam (ARVN) to combat an expected invasion from the North. These advisors didnt work as the communist Vietcongs seemed to be getting stronger and even though the numbers of advisors were secretly raised it didnt greatly help. The USA soon had to get even more involved as their strategic hamlet programme officials selected villages in South Vietnam which they thought could be easily defended by the Vietcong but didnt work as very few hamlets were secure and the policy was very unpopular with the Vietnamese. Religion was also a problem because Diem favoured Roman Catholics but this led to street protests like a Buddhist monk who set himself alight. Diem soon became increasingly unpopular and was assassinated by ARVN officers who ere worried that the USA would withdraw there support if Diem continued in power. However the assassination made things worse as Diems successors were no better at governing the country. This all led to the USA needing to involve themselves more by using the military. This first happened with the Gulf of Tongking incident which was an excuse for Johnson to get the Gulf of Tongking resolution which allowed him to take all the necessary measures to repel any armed attacks against the forces of the United States and to prevent further aggression. Then in February 1965 Johnson got the opportunity to justify bombing North Vietnam when the Vietcong attack the USAs base at Pleiku. Ten aircraft were destroyed eight US advisors were killed with over a hundred others wounded. Johnson could then call up troops and bomb the North. The operation was called rolling thunder. The planes bombed the key military and industrial targets of North Vietnam but were not allowed to bomb until he felt he could explain his actions to the American people. After that, 3,500 US marines were sent to the airbase of Danang and from then on the Americans took control of South Vietnamese war effort. This all generally happened due to the Americans obsession with communism and their belief of the domino theory, that once one country turned communist, so would the next and next which made the Americans more involved with Vietnam as time went by with the Americans trying to stop the spread of communism across South East Asia.

She encouraged the importance of disease prevention and taught health education. She provided well-baby care and treated minor illness in her main clinic named Henry Street Settlement House (Cherry, 2011). Lillian Wald emphasized the significance of prevention. A practice that continues to be reiterated everywhere. As seen in commercials or billboards or even on the radio, prevention is key to a long, healthy lifestyle. When patients leave the hospital, discharge education is vital to making sure they are capable to care for themselves and, hopefully, bring down the numbers of hospital visits. F. Scenario One principle I safeguarded for a patient was respect for autonomy.  Sometimes patients are admitted into the hospital for something as simple as daily intravenous antibiotics. They are independent with all of their activities of daily living. Sometimes these patients are intertwined with other patients in shared rooms. These independent patients, because of their alertness, should be able to be in a private room. These patients are not only of low acuity, but these patients will be happier not being around sicker patients. Happier patients means good rating for us. On the other hand, there are times when patients are admitted as stable yet confused. The second principle I safeguarded for a patent was non-maleficence. Because these patients are not in control of their thoughts or actions they are required to be supervised more closely than others. Patients who are at risk of getting hurt by getting out of bed without assistance can cause nurses to be of concerned. These patients can easily be put in a wheelchair and ‘hang out’ with the nurses at the nurses station for continuous supervision from, not only of the team nurse but, all staff. This would allow more eyes on the patient bringing down the risks of injury. This will also prevent more invasive means like using physical or chemical restraints.

Social workers wear many hats, and work with many different people, so therefore, they need to be experienced and knowledgeable in many different areas to effectively problem solve. Social workers must know how to work with people on the Micro, Mezzo and Macro levels. A social worker needs to understand different cultural values and the diverse differences in the people which they may be helping. They need to have a basic understanding of psychology, biology and public policy. They need to have an understanding of poverty and how it effects our society and what resources are available to those who might need them. A social worker should be very knowledgeable about the populations they are working with, what the needs of the client might be, what might help or hurt, and what resources may be available to better help those individuals or groups.
